HEALTH FIRST NETWORK, INC. HAS MOVED

Due to our current and future growth, HFNI has moved to a larger location.

If you need to come to our office, we are located at 3001 Langley Ave., Pensacola. This is just a few doors up the street from our old office and is the former site of Pensacourt.

We will let you know when we schedule our official Grand Opening!

URAC ACCREDITATION UPDATE

We are about to complete a significant URAC Accreditation milestone. Policies and Procedures for Core functions, as well as, Network and Credentialing, are almost complete. In October, these P&Ps will be uploaded to URAC for electronic (desktop) review. Based on this review, an on-site review will be scheduled to complete accreditation (targeting for some time in the December to February time frame).

With the addition of Care Coordinator resources this summer, good progress has been made on the two Quality Improvement Programs (QIPs) for managing fall and depression risks. Starting with the 2,200 patients who completed Patient Health Assessment (PHA) forms, approximately 35% have been assessed by the Care Coordinators. Of these, 73 and 106 have been assessed positive for falls and depression respectively. Notification of these at-risk patients has been sent to the appropriate PCP for treatment and some PCP’s have completed care for some of these. This intervention is important for patient safety in preventing falls or more serious depression; thus improving patient care and reducing additional cost (such as an ED visit or hospital admission). These QIP’s are on-going to meet needs of maintaining URAC Accreditation and for PQRS reporting.

CREDENTIALING

Health First Network, Inc.’s credentialing department will be utilizing CAQH as one of our information sources to help streamline the processing of initial credentialing, and re-credentialing applications. As A Primary Source Verification Organization, HFNI is responsible for collecting and verifying all required information, which includes CAQH data, but goes beyond what is available in CAQH. The additional information that HFNI asks for is required by most of the Health Plans to assure payment of claims. The following tips will be helpful in regards to the CAQH process:

► In filling out the data fields on the CAQH document, be as detailed as possible;
► Fill out ALL of the data fields, even if they are not marked as “necessary”, or are marked as “unnecessary”;
► Changes of address, telephone numbers, etc. should be recorded in CAQH as soon as possible;
► Review and attest to the correctness of the data at least every 4 months (this is actually a requirement by CAQH).

HFNI BOARD MEMBER, DR. BRUCE YOUNG, RETIRES FROM MEDICAL PRACTICE

Congratulations to Dr. Bruce Young on his retirement. Dr. Young is a board certified Family Practice physician and has been in private practice since September 1980. He is one of the founding members of Gulf Coast Physician Partners as well as Health First Network, Inc. Dr. Young has served on the HFNI Board of Directors since 2002. He will continue to maintain his leadership roles with Health First Network serving as Chairman of the Credentials Committee, as well as serving on the Board of Directors, Executive Committee, ACCNWFL Management Committee and ACCNWFL Compliance/Ethics Subcommittee.

Congratulations to Dr. Young on a job well done and a retirement well deserved.

ARE YOU AT RISK FOR A FALL

The 4th Annual Senior Falls Prevention Day was held on September 22nd at Santa Rosa County Auditorium. Free Screening consisting of Blood pressures, Blood Glucose, Balance Testing and many more were performed. The Health First Network team provided education on Fall Prevention and furnished no-slide socks to each participant. Door prizes were given away with lunch served. Those who came out greatly benefited and enjoyed the Falls Prevention Expo tremendously.

HEALTH FIRST NETWORK STAFF PARTICIPATES IN UNITED WAY'S "DAY OF CARING"

On Friday, October 10, 2014, Health First Network staff members joined more than 1,200 other local volunteers to participate in the United Way's "Day of Caring". The Day of Caring is a one-day, community-wide event that utilizes individuals from various businesses and military commands to administer an assortment of volunteer projects at non-profit agencies and schools in our community.

HFNI spent their day at The Retreat located on Royce Street. The Retreat offers a stimulating day experience for older adults with physical and cognitive impairments. HFNI volunteers helped participants enjoy a day of activities, social interaction and recreation.

COUNCIL ON AGING: RAT PACK REUNION

Council on Aging of West Florida (COA) will once again pay tribute to the original Rat Pack and honor a pack of local leaders at its Rat Pack Reunion fundraiser on October 24.
